Proof of immunization for MMR and TDap
- If you don’t have your childhood vaccine records, you will need to either have titers (blood test for immunity) done or be immunized again.
Current negative TB test
Proof of immunization for COVID-19
Copy of valid Driver’s License or Passport

Proof of annual Flu Vaccine in the fall
